Ruptur perineum merupakan keadaan dimana robeknya perineum pada saat janin
lahir. Data di BPM Bidan Y terjadi peningkatan kejadian ruptur perineum dari tahun
2015 sampai 2016 sebesar 3,1%. Tujuan penelitian, mengetahui gambaran faktor
penyebab kejadian ruptur perineum pada ibu bersalin di BPM tahun 2017. Manfaat
penelitian sebagai evaluasi bagi bidan untuk mendeteksi ibu yang memiliki indikasi
luka perineum sehingga dapat dilakukan upaya pencegahan saat proses persalinan.
Metode penelitian menggunakan studi kasus dengan pendekatan observasional,
subjek penelitian 6 orang ibu bersalin dengan ruptur perineum. Instrumen penelitian
menggunakan form isian dan lembar observasi. Data yang diperoleh disajikan dalam
bentuk narasi.
Hasil penelitian ke-6 subjek, 3 subjek mengalami ruptur perineum derajat I dan
3 subjek mengalami ruptur perineum derajat II. Ruptur perineum ini dikarenakan
faktor maternal yaitu kelenturan jalan lahir dan terdapat jaringan parut pada perineum
dan vagina. Faktor janin yaitu berat badan bayi. Dalam penatalaksanaan ruptur
perineum ke-4 subjek dilakukan penjahitan ruptur perineum menggunakan anastesi
sedangkan 2 subjek lainnya dilakukan penjahitan tanpa anastesi. Selain itu, dari ke -6
subjek penelitian tidak ditemukan adanya komplikasi yang terjadi akibat ruptur
perineum.
Kesimpulan penelitian yaitu terjadi ruptur perineum derajat I dan II, dengan
faktor penyebab terjadinya ruptur perineum adalah faktor kelenturan jalan lahir dan
jaringan parut pada perineum. Saran bagi bidan perlunya dilaksanakan asuhan sayang
ibu dalam penatalaksanaan penjahitan ruptur perineum dengan memberikan anastesi
dan deteksi komplikasi dilakukan dengan home visite pada ibu yang mengalami
ruptur perineum.

The perineum rupture is a situation where the rupture of the perineum at the
time of the fetus is born. Data at BPM midwife Y increased incidence of perineum
rupture from 2015 until 2016 as much as 3,1 %. The purpose of the study is to know
the decription of the factors causing the perineum rupture occurrence in mothers who
give birth in BPM 2017. The benefits of the study as an evaluation for the midwife to
detect mother who has an indication of the perineum wound so that prevention can be
done during delivery process.
The research method used case study with observational approach, the
subjects of research consist of 6 mothers give birth with perineum rupture. The
research instrument uses the form and observation sheet. The data obtained are
presented in narrative form.
Result of research of the 6 subjects,3 subjects had perineum rupture of grade I
and 3 other subjects had perineum rupture of grade II. This perineum rupture is
caused by maternal factor that are the flexibility of the birth canal and there is
carring tissue in perineum and vagina. Fetal factor is baby’s weight. In the
management of the 4 perineum rupture of the subject is conducted the suturing on
perineum rupture using anesthesia while 2 other subjects were sutured without
anesthesia. In addition, of the 6 subjects of the study, there was no complications that
occur due to perineum rupture.
This conclusion of this research there is rupture of perineum of grade I and II,
with the cause of perineum rupture is the flexibility factor of the birth canal and
scarring of the perineum. Suggestions for midwives that it needs to conduct the
mother’s love care in the management of suturing of the perineum rupture with
providing anesthesia and detection of complications conducted with home visit on
mothers who experienced perineum rupture.
